We will call "x" the number of small boxes and "y" the number of large boxes.

So the inequalities that model this situation are:

y ≥ 0 The number of large boxes can be any quantity.

5x + 12y ≤ 80 The total weight of the small(5x lb) and large(12y lb) boxes must be less than or equal to 80 lb.

x ≥ 4 The number of small boxes must be equal to or greater than 4 units.
