The slope-intercept form:

y=mx+b

m - slope

b - y-intercept

We have the slope
m=(1)/(2)\to y=(1)/(2)x+b

and point (4, 0). Substitute:

0=(1)/(2)(4)+b

0=2+b subtract 2 from both sides

-2=b\to b=-2

Answer:
y=(1)/(2)x-2
