The point-slope form:

y-y_1=m(x-x_1)\\\\m=(y_2-y_1)/(x_2-x_1)

We have the points (4, 8) and (-1, -12). Substitute:

m=(-12-8)/(-1-4)=(-20)/(-5)=4\\\\\boxed{y-8=4(x-4)}
