Answer:

Vertex form :
y=-2(x-4)^2+2

Intercept Form:
y=-2(x-5)(x-3)

Explanation:

We are given

y-2=-2(x-4)^2

Simplifying we get

Adding 2 on both sides

y=-2(x-4)^2+2

This is the vertex form

Now

Factoring -2 we get

y=-2[(x-4)^2-1]

y=-2(x-4-1)(x-4+1)

y=-2(x-5)(x-3)

is the intercept form
