The point-slope form of the equation of a line with slope m through point (h, k) can be written as

... y = m(x -h) +k

For your problem, where m = -4 and (h, k) = (2, 2), this becomes

... y = -4(x -2) +2

... y = -4x +8 +2 . . . . eliminate parentheses

... 4x +y = 10 . . . . . . .add 4x to put into standard form
