The Cubs, my dad's favorite ball club, finished in last place again.

You add the commas after 'Cubs' and 'club' because the middle information isn't really necessary, but you can add it in. 'my dad'd favorite ball club' is a dependent sentence whereas "The Cubs finished in last place again' is an independent sentence.
