Number of small boxes = x

Number of large boxes = y

she has at least 4 small boxes.

x \geqslant 4

Number of large boxes can be 0 or more.

y \geqslant 0

Maximum weight = 80lb

5x + 12y \leqslant 80

These are the required equations.

Your job looks correct. The number of small boxes is x, and we know that she has at least 4 of them so we have
x \geq 4.

We have no information about the number of large boxes, so the first two options make no sense.

Also, x boxes weighing 5lb and y boxes weighing 12lb sum up to
5x+12y, and this weight can be at most 80, so we have

5x+12y \leq 80
