According to OSHA studies, most LOTO-related accidents involved moving machinery. The primary causes of these types of incidents are:

Equipment not completely shut off before a maintenance or repair operation was performed

Machine turned on accidentally, either out of carelessness or by one worker not realizing another worker was in harm's way

Machine not working properly

Moving equipment wasn't properly chocked/blocked

Inadequate safety procedures

LOTO procedures had not been properly explained to workers

Even those workplaces that have established LO/TO processes face challenges, including: Lack of specific procedures written for each piece of equipment identifying all energy sources and energy isolation devices. Lack of comprehensive safety training for everyone in the workplace.
