I believe the correct condition is Down syndrome.
The components listed are key tell-tale signs of Down syndrome. Down syndrome is a result of incorrect chromosome pairing. The chances for babies to be born with Down syndrome can be passed down genetically and is more often in pregnancies of older women. Many who are diagnosed still live out very meaningful and influential lives and learn to push through their impairments.
I hope this helps! :)
