First, find the slope between the two points (3, 4) & (5, 16):

= 6
Then, input the slope and one of the points into the point-slope form:
y -
= m(x -
)
y - 4 = 6(x - 3)
y - 4 = 6x - 18
y = 6x - 18 + 4
y = 6x -14
Answer: y = 6x - 14
