First, find the slope between the two points (3, 4) & (5, 16):
m =
( y_(2) - y_(1) )/( x_(2) - x_(1) )
=
( 4 -16)/( 3 - 5)
=
(-12)/( -2)
= 6

Then, input the slope and one of the points into the point-slope form:
y -
y_(1) = m(x -
x_(1))
y - 4 = 6(x - 3)
y - 4 = 6x - 18
y = 6x - 18 + 4
y = 6x -14

Answer: y = 6x - 14
