Elbow restraints have been prescribed for a confused client to keep the client from pulling out a nasogastric tube and indwelling urinary retention catheter. What is most important for the nurse to do?

it is important for the nurse to keep the patient calm, as agitation and anxiety can cause them to pull it out. i would think it is also important to monitor neurological function, because if a patient goes from drowsy to (at least somewhat) alert, they can also pull it out. at that point, the nurse can begin to calm the patient as he regains his consciousness.
