y-9=12(x-5)

Explanation:

Point Slope Form: y-y1=m(x-x1)

Where m=slope and (x1, y1) is a point on the line.

m=(y2-y1)/(x2-x1)=(-3-9)/(4-5)=-12/-1=12

So the slope is 12,

y-9=12(x-5)
