25 + 12x

Explanation:

25 is the initial sign up fee and 12 is the cost per month.

When you see the words 'one time fee' never worry about putting a letter behind it. It will never change.

When you see the words 'yearly', 'monthly', 'daily', etc,. you will need to put a letter behind it because it will change depending on the number of years, months, days, etc,.

If you take what I said into account, you will end up with this equation:

25 (initial fee) + 12x (where x is the number of months)
