(y-y0) = m (x-x0)

So,here the equation with m = 5 and point (1,5) is given as:

y - 5 = 4(x - 1)

or, y - 4x = 1

So, the SLOPE INTERCEPT form of the equation is y = m x + C

or, y = 4 x + 1 , slope = 4, intercept = 1
