We find the slope by dividing changes in the y coordinate by the changes in the x coordinate:

(-5-0)/(2-1)=(-5)/(1)=-5

So, our line comes in the form

y=-5x+b

Plug in one of the (x,y) pairs from the table to get the value of b. For example, if we use the first row we have

0=-5\cdot 1+b \iff b=5

So, the equation of the line is

y=-5x+5
