It is given by:

where,
$ (x_1, y_1) $ is the point that passes through it and
$ m $ is the slope of the line.

Substituting we have:
$ y + 4 = (-1)/(6)(x - 6) $


\implies 6y + 24 = -x +6


\implies x + 6y + 18 = 0
