First remember that this is asking for slope-intercept form which is y=mx+b, where m is the slope and b is the y-intercept. To solve this problem you must find the slope. Do this by either counting the increase of y and then putting that over the increase in x, or you can use the formula. The slope formula is
(y_(2)-y_(1) )/(x_(2)-x_(1) ), so plugin any two points and solve. No matter which way you solve the slope is 3. Then remember the y-intercept is the value at which the line crosses the y-axis, in this case, the line crosses over at the point 6, so that will be the b value. This means the final equation will be y=3x+6.
