y=0.25x+2

Explanation:

Attached is a picture of point-slope form. m=slope

We already have been given the slope 0.25

1) Plug in 0.25 as m in point-slope form:

y-y₁=0.25(x-x₁)

2) Now plug in the points as x₁ and y₁ respectively:

y-5=0.25(x-12)

3) Distribute 0.25 to x and -12:

y-5=0.25x-3

4) Add 5 to both sides:

y=0.25x+2

- If you are only required to answer in point-slope form, the answer would be y-5=0.25(x-12)
