\huge\boxed{12x-4y=-3}

Standard form is
ax+by=c, where
a,
b, and
c are real numbers. The requirements vary a little bit from textbook to textbook, but I will follow the most common rules.

We are starting off with an equation in slope-intercept form.
y=3x+(3)/(4)

Subtract
3x from both sides.
-3x+y=(3)/(4)

Multiply both sides of the equation by
4 to get rid of the fraction.

Multiply both sides of the equation by
-1.
\boxed{12x-4y=-3}
