[tex]r = \sqrt(x^2 + y^2)[\tex]

theta = arctan(y/x)

So for your pair:

[tex]r = \sqrt{(2\sqrt{2})^2 + (2\sqrt{2})^2} [\tex]

[tex]r = \sqrt{8+8} [\tex]

[tex]r = \sqrt{16} [\tex]

r = 4.
