From question statement ,we observe that

slope=m= 5

y-intercept at (0,-9) which means c=-9.

Putting the value of slope and y-intercept in slope-intercept form,we get

y=(5)x+(-9)

y=5x-9 is equation of line that passes through (0,-9) and has a slope equal to 5.
