= (12-4)/(1--3)

= 8/(1+3)

= 8/4

=2

The slope is 2

We can use the point slope form of the equation

y-y1 = m(x-x1)

y-4 = 2(x--3)

y-4 = 2(x+3)

Distribute the 2

y-4 = 2x +6

Add 4 to each side

y-4+4 = 2x+6+4

y =2x+10

Now this is in the form y= mx+b
