The answer to your question is just use Desmos

Answer: y = 5/4x - 1 , Slope = 5/4 , y-intercept = -1


STEPS:

Use y1-y2/x1-x2 to find the slope

-1-4/0-4

= -5/-4

= 5/4 because negative divided by negative is positive


Next, use y-y1=m(x-x1) to find y-intercept


y-4=5/4(x-4)

y-4=5/4x-5

y=5/4x-1

So the y-intercept is -1
