In general, the slope-intercept form of a linear equation is


\begin{gathered} y=mx+b \\ m,b\rightarrow\text{ constants} \end{gathered}

Thus, in our case,


\begin{gathered} y-2=-5(x-2) \\ \Rightarrow y-2=-5x+10 \\ \Rightarrow y=-5x+12 \end{gathered}

The answer is y=-5x+12
