78 yo M has 3 day hx lethargy&confusion. 2 episodes of vomiting over last 12 hrs.Had A stroke 5 yrs ago that resulted in dense left-sided hemiplegia. Urine incontinent. T 101.2F. HR 102. Dry oral mucosa. WBC 16500,sodium 124. UA large leukocyte esterase, 50-100 wbcs. Tx?
