Explanation:

M = slope

B = y-intercept

The slope is found by counting the rise over run between two points.

For example, between (0,4) and (3,5), the second point is 1 up (rise) and 3 to the right (run). Therefore, the slope is 1/3

The y-intercept is what point hits the y-axis, so 4

Now you would just plug those quantities into y = mx +b
