The given polynomial :

12x+4x^2-x^5+3

Standard form of polynomial :

Polynomials in is called standard form when the terms are ordered in highest degree to lowest degree.

In the given polynomial the highest degree is 5, then 2, then 1

So, the polynomial is arrange as :

12x+4x^2-x^5+3=-x^5+4x^2+12x+3

The standard form of the given polnomial is ;

-x^5+4x^2+12x+3

The leading coefficient is the coefficient of the hoghest degree variable.

Here the highest degree is 5 and the coefficient related to x^5 is (-1)

So, Leading coefficient = (-1)

Answer : Standard Form :

-x^5+4x^2+12x+3

Leading coefficient = (-1)
