pont slope is
given the point (x1,y1) and the slope being m
y-y1=m(x-x1)

we are given
y=6x+9
slope=6
y intercept is 9
so the slope is 6 and a point is (0,9)

y-9=6(x-0) is a point slope form (there are infinite)
another would be
y-15=6(x-1)

d. that is just y=6x+9
