I Believe the answer is: A 50-year-old woman who is reporting increased irritability for the past 2 months. She states that she goes to bed at 10 p.m. every night and tries to sleep in but, no matter what she does, she always wakes up around 4 a.m.

A person could be considered to have an insomnia if that person already make the effort and provide enough time to gain suffifcient sleep but unable to do so. You could see it from this part of the description :
she goes to bed at 10 p.m. every night and tries to sleep in but, no matter what she does, she always wakes up around 4 a.m.
