I believe that the answer is foreshadowing because Fortunato has a problem in which he may die. According to the book, Johnny Tremain, Johnny's mother coughed herself to death, showowing or suggesting that Fortunato may do the same. He may die of a cough, some ay say that this results to foreshadowing.
