I believe the answer is D.Three.
The majority of the cases came from elderly that never got insurance and the fact that people who took insurance tend to be more cautious in taking care of their health.
As we got older, the chance of our body receiving complications of illness would be increased. Since insurance wasn't really considered as important as it is at that time, majrotiy of the old timer never really covered by any insurance.
