1. The slope-intercept form is y=mx+b and therefore you have to find out the gradient of the slope (m) and the y-intercept (b).

You should first plot out the points given to you on a number plane, ie at x=-2 the corresponding y-value is at 5.

2. Then use rise/run to find the gradient of the slope:

8/4=2.

NOTE: However, since the slope is negative the gradient is also negative, therefore it is -2.

3. The last thing you have to find is the y-intercept, which in this case is when x=0. From the given values, when x=0, y=1.
