Slope intercept form is y=mx+b. Where m is your slope and b is your y-intercept. For #11 you have a slope of zero,you have a horizontal line at (0,-5) so any horizontal line above or below (0,-5) would give you a parallel line to y=-5. For #12 your slope if -3, any equation with the same slope will give you a parallel line.
