Becky just delivered a 9 lbs. baby boy. She had a vaginal delivery under epidural anesthesia. Noted in report she also experienced an episiotomy. She is calling from her room because she is hearing something dripping and it is keeping her awake. Noted in report, she has not gotten all the feeling back in her legs. When you arrive, there is no water dripping. You notice a puddle of blood under the bed. You remove the blankets and you see blood behind her, and the blue sheets saturated. What is your initial care, and then the nursing interventions you would provide.

A uterine revision is initially done to rule out that it is a hypotonia and that it requires urgent medical management, by the doctor on duty, by the history of macrosomal vaginal delivery. It should also be ruled out that it is not about placental remains or any accompanying alteration of coagulation factors. It must be assessed by the doctor on duty, request blood, prior hemoclassification, cuagulacion factors, among others.
