The question is a bit weird, but I think the best answer would be D ”Are you sure you want to do that?” He asked with a sly, insinuating smile

You are not even sure what it means. What exactly is he half warning someone else about? And why is he being sly and insinuating when he makes a statement that hardly warrants any notice at all? You are revealing something about the speaker, at least, that perks up our ears, but we're not sure why.
