Any of the first three can be great stress provokers. Withdrawing from substance abuse is pretty awful. It can be physically painful as well as being with you for a long time (like a month).
The loss of someone is also very stressful if it is a long standing relationship.

Of all four of them, I would pick D. It is an individual matter, and if you are prone to feel stress, you will feel it deeply, no matter what it is.
